About your organization

Organization lead name (e.g. chair or president)*
Organization lead email*

See this list for account numbers (both OSL and AS are fine). If you don't have an account number or can't find it, just leave this blank.

Having an account number reduces paperwork and lets you get your money faster, since it lets us directly transfer funds into your account (in what's known as a journal entry) rather than having to mail you a check. If you request funds from AS organizations often, consider opening an AS account to get a number.

IVCRC's current policy is to issue checks to non-Registered Campus Organizations whether or not they have an account number. This is because there are additional steps required for off-campus organizations to access their funding if it is transferred via a journal entry, which can be very inconvenient.

Please enter a number from 0 to 999.

About your project: the basics

A name makes it official. It doesn't have to be fancy—it just has to be something we can both refer to easily.

If it's an event, where will it be held?

If it's more of a long-term program or service, where will people need to be in order to access it?

If it's an event, when will it be held?

If it's a program or service, when will it start?

Either way, an approximation is fine.

If it's an event, when's the last day? (It might be the same as your start date.)

If it's a program or service, when will it stop, or when will you spend all your funding?

Either way, an approximation is fine.
